What is the ACL?

The Anterior Cruciate Ligament (ACL) is a key stabilizer of the knee. It connects the thigh bone (femur) to the shin bone (tibia) to prevent the shin from sliding forward and twisting excessively.

How ACL Injuries Happen

Most ACL injuries (70% to 80%) happen without direct contact:

  • Sudden stops while running
  • Quick changes of direction (cutting)
  • Landing awkwardly from a jump
  • Twisting with the foot planted

Types of ACL Injuries

  • Grade 1 (Mild): Ligament stretched, not torn; knee remains stable.
  • Grade 2 (Partial Tear): Some fibers torn with mild instability.
  • Grade 3 (Complete Tear): Ligament is fully torn; knee is unstable and often requires surgery.

Signs and Symptoms

  • Loud “pop” sound at the time of injury
  • Sudden, sharp knee pain
  • Rapid swelling within hours
  • Knee feels unstable or gives way
  • Difficulty continuing activity or bearing weight

Treatment Options

  • Non-surgical: RICE protocol, knee brace, and physical therapy for low-demand individuals or partial tears.
  • Surgical reconstruction: The torn ligament is replaced with a graft, followed by structured physical therapy.

Recovery Timeline

  • 1 to 2 Weeks: Reduce swelling and pain.
  • 3 to 6 Weeks: Restore full range of motion.
  • 2 to 3 Months: Build strength and joint control.
  • 4 to 9 Months: Sport-specific agility training.
  • 9 to 12 Months: Full return to pivoting sports with clearance.
