Calcific Tendinitis of the Shoulder

A treatable cause of severe shoulder pain caused by calcium crystal deposits inside rotator cuff tendons.

What is Calcific Tendinitis?

Calcific tendinitis (calcific tendinopathy) happens when calcium crystals build up inside a tendon. These deposits irritate surrounding tendons and bursae, causing inflammation, pain, and limited movement.

Approximately 80% of cases occur in the shoulder, most commonly affecting the supraspinatus tendon, followed by the infraspinatus and subscapularis.

Note: It is not caused by dietary calcium intake or taking calcium supplements.

Who Gets It and What Causes It?

  • Most common in adults between 40 and 60 years old, with women slightly more affected than men.
  • Contributing factors include reduced blood supply to the tendon, natural aging, cellular changes, diabetes, thyroid disorders, and hormonal factors.

Common Symptoms

  • Sudden, severe shoulder pain
  • Pain when lifting the arm or reaching overhead
  • Night pain and difficulty sleeping on the affected side
  • Stiffness and reduced range of motion
  • Tenderness and localized weakness

Diagnosis and Treatment

Diagnosis

  • Physical examination: Checking range of motion, tenderness, and strength.
  • X-ray: The primary imaging test for identifying calcium deposits.
  • Ultrasound and MRI: Used to assess deposit size, location, and surrounding tendon health.

Treatment

  • Rest, activity modification, ice, and NSAIDs.
  • Physical therapy for mobility and rotator cuff strength.
  • Corticosteroid injections or ultrasound-guided barbotage (lavage).
  • Extracorporeal shock wave therapy (ESWT).
  • Surgery is needed only in a small percentage of persistent cases.
